Diabetes information and support in Brent
GP support
- NHS Diabetes Prevention Programme (NDPP): identifies those at high risk and refers them onto a free behaviour change programme. This can be accessed through a GP.
- Eye check: Anyone over 12 and with diabetes should get their eyes checked at least once a year. Speak to a GP for support to get eyes checked.
- Foot checks: If someone has diabetes then they should regularly check their feet for cuts, redness, swelling and other foot conditions. They should see a healthcare professional if they see any changes.
- Planning pregnancy with diabetes: A healthcare professional can provide support with pregnancy planning for people with diabetes
- Glucose monitors: Some patients with diabetes need to test their blood glucose. Speak to a GP to see if you are eligible.
- Brent Integrated Diabetes Service: The service focuses on Type 2 diabetes patients in primary care and community settings. They offer diabetes clinics, Link diabetes nurses, self-management educational classes and more. A person can access them through GP referral.
Brent Council
The Brent Diabetes Community Champions raise awareness about the signs, symptoms and risk factors of Type 2 diabetes and how individuals can reduce their risk and manage their diabetes.
They are also carrying out “Know Your Risk” assessments in the community and signpost people to local services.
